OBJECTIVES: To assess the efficacy of 67Gallium lung scan at the early
detection of the Pneumocystis Carinii Pneumonia (PCP). METHODS: 10
patients have to be studied (9 males, 1 female; median age 28, 6IVDU, 2
homosexual men, 2 heterosexual transmission) without respiratory
symptomatology, with CD4 less than 500 mm (ranged from 30-450 with a
mean of 124) and with a normal chest radiography. All of them underwent
a 67Gallium lung scan. RESULTS: 2 patients had a bilateral diffused
imaging, for which reason they underwent a bronchoscopy, so it remained
apart in the bronchoalveolar lavage pneumocystis carinii. One month ago
the first patient showed an episode of cerebral toxoplasmosis with a
quantity of CD4 77 and Pa O2 97. The second patient showed a Kaposi's
Sarcoma with a quantity of CD4 60 and Pa O2 95. CONCLUSIONS: In
accordance with these information, we believe that 67Gallium lung scan
would be useful for patients without respiratory symptomatology with a
normal chest radiography, for those who have a diagnosis of disease
associated with infection because of the HIV and/or those who have the
quantity of CD4 less than 200. There is another point to determine, that
is the relation cost-benefits of the method 67Gallium lung scan at the
early detection of PCP.
