INTRODUCTION. To date it is unclear whether the specific profile of p24 Ag, which seems to be only detected before and shortly after appearance of HIV antibodies and at the terminal stages of infection and development of AIDS, is due to low or absent synthesis of viral antigen or to the formation of immune complexes. MATERIAL AND METHODS. We have studied the presence of CIC Ag (p24 Ag in immune complexes) in 59 sera from 43 HIV seropositive patients and 10 sera from HIV seronegative subjects. Sera were treated with PEG to precipitate CIC and guanidine to dissociate them. p24 Ag assays were performed by EIA (PASTEUR); sera were also tested for the presence of anti-HIV antibodies by EIA (ABBOTT) and Western blot (PASTEUR). Quantitation of anti p24 antibodies was established by EIA (SORIN). RESULTS. After HIV-CIC dissociation with PEG/Guanidine treatment the p24 Ag was detected in 54.2% serum (vs. 13.5% untreated); p24 Ag was not detected in seronegative sera with the same treatment. CIC Ag was detected in patients at all stages of infection but was most prevalent in asymptomatic subjects and in individuals with high levels of anti-p24 antibodies. CONCLUSION. For the follow up of HIV infection, it is necessary to test for CIC Ag and not only for free p24 Ag.
