ACCELERATING DECLINE IN NEW YORK CITY AIDS MORTALITY

OBJECTIVE AND BACKGROUND: To compare AIDS mortality in NYC for the 2nd half of '96 and the 1st half of '97 by gender, race/ethnicity, and age. Comparisons by HIV transmission category are for '95 and '96. AIDS deaths declined 29% from 7,046 in '95 to 4,998 in '96, the first drop since the beginning of the epidemic.

METHODS:The analysis includes all NYC death certificates filed with NYCDOH by 9/11/97. Data for the 1st half of '97 are provisional, but only small changes are expected. ICD-9 codes 042-044 were used to identify AIDS deaths. AIDS case surveillance data were used for estimates of mortality by risk behavior.

RESULTS: AIDS deaths fell 33% (2,118 to 1,423) from the 2nd half of '96 to the 1st half of '97. Deaths declined 33% (1,517 to 1,016) for men and 32% for women (601 to 407). Deaths fell for both men (M) and women (W) by race/ethnicity: whites, 41%M & 33%W; blacks, 28%M & 30%W; and Hispanics, 34%M & 37%W. The largest change was seen in black women whose 30% decline nearly doubled from 16% between the 1st and 2nd half of '96. Deaths also declined in all age groups: 45% in 0-14 yrs.; 39% in 15-24 yrs.; 36% in 25-34 yrs.; 35% in 35-44 yrs.; and, 28% in 45+ yrs. By risk, deaths fell approximately 40% in MSM and approximately 30% in IDU and heterosexuals from '95 to '96.

CONCLUSION: The dramatic decline in AIDS deaths first observed in NYC in 1996 continued through the 1st half of 1997 in all demographic groups. This occurred at a time when new antiretroviral therapy became widely available.
