BACKGROUND: The year 2006 is year 25 of HIV, year 20 of ART, and year 10 of HAART.

METHODS: The presentation has 3 parts: The major events of the past decade, a brief statement of where we are now, and then predictions of where the field will go.

RESULTS: Highlights will include: the incremental gains that have led to 3,000,000 life-years saved in developed countries, initiation of efforts to apply these lessons to developing countries, and a multitude of lessons learned en route. The disappointments are the near total failure in prevention, lack of vaccine, and lack of a cure.

CONCLUSIONS: The future will bring new drugs from one of medicine’s richest pipelines, simplified regimens, opt-out testing, a healthcare infrastructure in developing counties, and the potential for 30,000,000 life-years saved.
