OBJECTIVES: To prepare clinical trial sites and develop capacity by conducting research to obtain baseline prevalence, estimates of incidence and risk factors in populations willing to receive voluntary counseling and testing (VCT) at four sites in Uganda and Kenya.

METHODS: Four sites, selected for development as potential HIV vaccine efficacy trial sites, were provided community education, developed field mobile/static research VCT clinics conducted a cross sectional survey of HIV prevalence in volunteers aged 18-60 yearsRisk factor and demographic data were collected. Viral typing and the BED assay to obtain estimates of incidence are in progress.

RESULTS: Between January and December 2004, 6,463 volunteers who were willing to have VCT were recruited at four sites. The prevalence data, by gender, is summarized:

At all sites HIV prevalence peaked in the mid 30's. Women became infected more frequently and at younger ages with the greatest slope between 18 and 30 years of age.

CONCLUSIONS: Conduct of this prevalence study has prepared the community for research and VCT in addition to providing some baseline demographic and risk data that will allow for the selection of subgroups of potential volunteers at higher risk for HIV vaccine efficacy trials and strengthen referral networks for prevention and care.
