SERUM TOTAL GHRELIN ASSESSMENT IN HIV-1 INFECTED PATIENTS WITH AND WITHOUT ANTIRETROVIRAL DRUG TREATMENT

AIM: To study HIV-1 infection influence of on total serum ghrelin and to assess HAART impact on serum ghrelin, body composition and appetite.

MATERIAL AND METHODS: Twenty controls and 35 HIV-1 infected patients (20 on HAART and 15 with no antiretroviral therapy), all males with age between 20 to 40 years old, with assessment of total serum ghrelin (RIA), body composition (bioelectrical impedance) and appetite (visual analogue scale).

RESULTS: No age differences among groups were found. Controls had a higher BMI [controls: median 27.3 (P25–75 24.4–30.1); HIV patients on HAART: 23.4 (20.9–26.4); HIV patients with no HAART: 23.8 (22.6–27.7); P=0.009]. No significant differences among groups concerning serum total ghrelin were found although HIV patients on HAART showed a higher circulating level [controls: 434.6 pmol/l (342.0–508.9); HIV patients on HAART: 471.7 pmol/l (390.9–603.8); HIV patients with no HAART: 389.1 pmol/l (328.3–528.2); P=NS]. Also no significant differences in appetite among groups were found although HIV patients on HAART exhibited higher scores [controls: 57.0 mm (51.8–66.5); HIV patients on HAART: 72.0 mm (50.8–83.3); HIV patients with no HAART: 53.0 mm (35.0–75.0); P=NS]. Fat mass was higher in controls and HIV patients with no HAART [controls 21.9 (18.6–25.6); HIV patients on HAART: 12.3 (11.3–19.1); HIV patients with no HAART: 19.3 (12.6–22.3); P=0.002].

CONCLUSION: HIV-1 infection does not appear to influence total serum ghrelin circulating levels. HAART is associated with a trend to higher appetite scores and an increase in total serum ghrelin.
