The treatment of Kaposi's sarcoma with cytotoxic agents has many controversies. Although a number of agents including the vinca alkaloids such as vincristine and vinblastine, etoposide, adriamycin, bleomycin and liposomal doxorubicin, have been shown to give significant response rates, both partial responses (PR) and complete responses (CR), these responses are often mixed and transient. Controversies in the treatment of Kaposi's sarcoma with chemotherapy also extend to when to introduce drugs and whether chemotherapy should be given in isolation or with other modifiers such as interferon. Combination chemotherapy, particularly adriamycin, bleomycin and vincristine (ABV) have been given with adequate response rates but are often associated with significant toxicity. The use of cytokines such as G or GMCSF to allow adequate dose increments has been associated with hematological tolerance but has not yet been reported to give better response rates in published trials. As well, many patients presenting with end stage HIV illness and KS are on a multitude of other drugs which limit adequate delivery of cytotoxics.

In the light of the round-table discussion of other types of treatment,the chemotherapy of KS will be discussed both in terms of its indications and pitfalls. New cytotoxic agents such as paclitaxel are also under investigation and will be discussed.
