Approximately 90% of global HIV infections have occurred in citizens of resource-poor countries; over 12.5 million persons are living with HIV in non-industrialized countries. Emphasis on prevention in the early stages of the HIV/AIDS epidemic in resource-poor areas is now tempered by realization of the need to address appropriate, affordable levels of care. Research is required for better understanding of the natural history and spectrum of disease of HIV/AIDS in resource-poor countries, including the nature, timing and frequency of the major opportunistic diseases.
Gynaecologic and paediatric aspects of HIV disease have been particularly neglected. The efficacy of prophylactic therapy against specific diseases such as tuberculosis and toxoplasmosis needs to studied, as well as the feasibility of their implementation. Therapy for specific conditions needs to be evaluated, and drug requirements determined. Better definition of overall needs for care of persons with HIV/AIDS, including nursing, psychosocial support, and terminal care is required. Attempts are required to limit the widening gap between interventions available for HIV-infected persons in different parts of the world.
