Failure to achieve complete viral suppression by antiretroviral treatment is common in clinical practice, occurring at a high rate in many cohorts. The reasons for drug failure are multi-factorial but resistance to current medications is recognized to be an important element. While waiting for new drugs with activity against resistant virus, methods for measurement of drug resistance have been introduced into clinical practice in order to optimise the remaining therapeutic options. The use of such tests has been supported by international guidelines. However almost all prospective studies have evaluated the clinical utility of resistance testing based on short-term virologic responses. In addition, the published results have provided somewhat conflicting data. Although there is a detailed knowledge about mutational patterns of different drugs, cross-resistance patterns and about technical aspects on resistance testing several clinically important questions remain to be answered. Identification of clinically relevant phenotypic susceptibility cut-offs for approved drugs, comparative data between different assays and test interpretations systems may possibly contribute to improved clinicial utility. Importantly, even if short-term virologic benefit can be obtained the long-term impact on immunological parameters and clinical outcome remains unexplored.
