Since the Centers for Disease Control began surveillance for AIDS in 1981, 72 cases of AIDS in children under 13 years of age have been reported. The childhood cases differ epidemiologically from those in adults: 74% (53) of the 72 children are Black or Hispanic, compared with 39% of adults; and 58% (42) of the children are male, compared with 94% of adults. Children with AIDS have been reported from 12 States, with 81% (58) residing in New York, Florida, New Jersey, or California. Of the 72 children, 65% (46) have one or both parents who have AIDS and/or are members of the known high-risk groups for AIDS, including bisexual men, intravenous drug abusers, Haitians, or hemophiliacs. The median age at time of diagnosis in these children of high-risk parents is 7 months compared with a median age of 13 months in the 12 children who developed AIDS following blood transfusions in the neonatal period. This suggests that transmission in those children of high-risk parents most likely occurs in utero. Of these 12 transfusion-associated (TA) cases, 83% (10 cases) occurred in boys, compared with 54% of cases (3/2/59) in other children. Three of the 12 children with TA AIDS were twins.
