In 1983-84 we interviewed 246 successively-diagnosed, CDC-defined, homosexual AIDS patients in San Francisco, 44% with KS only and 56% with 0I or both diseases. 165 patients were success-fully matched with randomly-chosen homosexual men picked as neighborhood controls; 166 were matched to VD clinic controls. We report a paired, case-control analysis of AIDS risk factors associated with lifestyle, sexuality, drug use, and infectious disease history. Comparing cases with clinic controls we found small differences, probably because many clinic controls have been exposed to the AIDS virus. Comparing with neighborhood controls we found (a) a strong association between AIDS risk and number of sexual partners, (b) a moderate association with anal receptivity which persisted when controlled for number of partners, (c) no association with oral receptivity, (d) no association with drug use, including "poppers" either for all AIDS or for KS only and (e) a moderate association with use of bathhouses and clubs which was confined to persons with smaller numbers of partners. Sera from all controls were examined for antibodies to the LAV virus. 33% of neighborhood controls and 66% of clinic controls were abs. We report a companion case-control analysis based on seronegative controls only.
