DIAMINODIPHENYLSULFONE (DAPSONE) AS TREATMENT FOR PNEUMOCYSTIS CARINII PNEUMONIA IN AIDS PATIENTS IN COMBINATION WITH TRIMETHOPRIM

The high frequency of adverse reactions to conventional therapy of Pneumocystis carinii pneumonia (PCP) in AIDS patients warrants a search for an efficacious, less toxic agent. Dapsone has been shown to be effective in a rat mxlel. We used Dapsone (100 mgs p.o./d.) and Trimethoprim (20 mgs/kg/d) for two weeks in a patient in when other treatment was contraindicated. Symptomatic aril radiologic resolution of pneumonia occurred within 7 d. One year later, PcP was not found at autopsy. Six more PCP/AIDS patients have received this treatment. Five had clinical resolution of PCP. No relapse has occurred (mean follow-up-3.6 mos.). Two patients had improvement but therapy was changed due to side effects; pancytopenia and hemolytic anemia. Other side effects include rash, 3 and pancytopenia, 1. Treatment of PCP with Dapsone and Trimethoprim appears to be effective but a high rate of side effects (71%), sane severe, may limit its usefulness.
