Four homosexual men with AIDS and culture positive CMV infection were treated with DHPG, 5 mgm/kg. I.V. 3 times daily for at least 10 days. Antiviral effect was demonstrated as follows:

Patient #1 received 2 separate 14 day treatments and subsequent intermittent single doses. More intense therapy was prevented by severe neuropenia. His last dose was 5 weeks prior to death. Patient #2 received DI-PG for 14 days and then 3 times per week until his death from P. carinii pneumonia. Patient #3 received 10 days of DHPG. Prior to treatment he was blind in one eye and had severe retinitis in the other. No autopsy was performed. Patient #4 also received 10 days of therapy but expired on the 10th day from his 3rd episode of P. carinii and CMV pneumonia. All 3 of the patients with retinitis had subjective and objective improvement in conjunction with a diminution in viruria, but patient 3's retina was culture positive at autopsy. Autopsy culture results for patient 1,2, and 4 were as follows:

We conclude that DHPG effectively suppresses CMV excretion and appears to provide clinical benefit for CMV retinitis. It does not eradicate CMV in severely immunocompromised AIDS patients.
