We have followed a cohort of 113 homosexual men for up to 18 months. At enrollment 28/57 healthy homosexual controls, 36/40 patients with AIDS related complex (ARC), and 17/17 patients with AIDS were HTLV-III seropositive (HTLV-III +). HTLV-III + donors had fewer T4 (596±37 vs 943±60/mm³ p<.005) and more la+ lymphocytes (19.7/2.5 vs 11.of 0.7%, p<.001) than healthy HTLV-ITI - control donors. The 6 of 28 healthy HTLV-III + individua3s who developed ARC had fewer T4 (432f94 vs 621/38/mm³, p<.025) and more Ia+ lymphocytes (33±14 vs 17±1.7%, p<.05) at enrollment than did those who did not develop ARC. The 5 of 36 HTLV-III + ARC patients who progressed to AIDS had fewer T4 lymphocytes (114/48 vs 582/75/mm³, p<.01) and a lower Con A response (5391±1871 vs 41509±7560 cpm, p<.05) than did those who remained stable. Immunologic parameters of stable ARC and healthy HTLV-III + patients were generally unchanged for the first year of the study. One of 2 patients who became HTLV-III + after enrollment has developed progressive depletion of T4 lymphocytes over the 9 months since seroconversion.
This study further defines the natural history of HTLV-III infection, and may be of use in planning treatment trials and in patient counseling.
