OBJECTIVE: To determine the prevalence of HTLV-I/II infection in Spanish populations at high-risk for HIV infection.

METHODS: Serum samples were obtained from 150 subjects (1987-88) distributed in 110 drug users, 15 heterosexual contacts, 22 homosexuals and 3 post-transfusional HIV infections; 93% of them were anti-HIV+ and 10% were symptomatic (ARC/AIDS). Anti-HTLV-I were tested by 3 licensed EIAs (Abbott, Dupont, Cellular Prod.). Repeatedly reactive (RR) samples were confirmed with WB, RIPA and a recombinant p21E (env) WB (Biotech). A + result was defined as the presence of p24 and an env product in any of the 3 tests.

RESULTS: Nine samples (drug users) were RR in at least one of the EIAs: TABULAR DATA, SEE ABSTRACT VOLUME. Only 4 samples (2.6% drug us.) were interpreted as + (No.1, 2, 4, 7). None of the EIAs was + in all 4 samples. All + samples were detected by the recombinant WB assay.

CONCLUSION: 1) The prevalence of HTLV-I/II infection in HIV infected groups in Spain is very low; 2) Additional confirmatory testing for HTLV-I/II infection is absolutely necessary; 3) The recombinant p21E WB assay might be a useful confirmatory technique for HTLV-I/II infection.
