OBJECTIVES: To determine the prevalence of HIV infection among persons at risk of HIV infection at Cook County Hospital, Chicago, in 1987 and 1988, and to compare trends among groups at risk.

METHODS: From 1/1/88 to 12/31/88, 2,084 clients received HIV antibody counseling. Demographics: 42% women, 58% men; 69% black, 15% Hispanic, 15% white. Of 2,084 persons counseled, 1,146 (55%) were tested in 1988; in 1987, 1,042 people were counseled and 522 (50.0%) were tested. Two positive EIAs and a positive Western Blot were considered positive. Class IV HIV infection was excluded.

RESULTS: Table I shows the results: TABULAR DATA, SEE ABSTRACT VOLUME. Of those positive in 1988, 101 (82%) were male, and 31 (18%) female. By race, 181 (66.5%) were black, 55 (20.2%) Hispanic, and 34 (12.5%) white. Four (1.5%) were newborn & 31 (11.4%) were 18-24.

CONCLUSIONS: Overall, a 30.7% increase in HIV seropositivity was observed from 1987-1988. Increased access to HIV counseling and testing resulted in a 178.3% increase in HIV seronegative persons at risk counseled. Increased numbers of infected IVDUs and heteros were identified.
