Taking care of prisoners infected by H.I.V. or suffering from AIDS and training of personnel.

OBJECTIVE. The objective of the Programme is to propose systematically a test to all prisoners and to take care clinically, biologically, and psychologically of those infected by H.I.V. or suffering from AIDS, including medical follow-up when they come out, in Fleury Merogis, where the largest prison in Europe is located. METHOD. The penitentiary administration allows prisoners to have access to appropriate medical care provided by adequate staff, technical material and premises. The Hospital Pitie Salpetriere delegates two specialised physicians who attend, upon request of the prison medical team composed of 17 physicians and 20 nurses, and offers the day-time hospital structure, hospitalisation and extern consultation. All warders in duty or in training, receive specific compulsory training on AIDS in order to improve their taking care of incarcerated patients. RESULTS. The prison of Fleury Merogis hosts 5,000 prisoners. Among them, 1,025 are seropositive and 72 have AIDS, in an advanced stage of the disease, under Zidovudine treatment (data on January 21, 1991). A prophylaxy has been set up for all patients whose CD4 cells number is below 200: bactrim (septrim) vs. pentacarinate nebulization. Once they are free, 1 out of 5 come again spontaneously to consult at Hospital. CONCLUSIONS. It seems essential to consider separately the struggle against AIDS from the general context of prison, to associate medical care provision and information of all prisoners, to rely on all the personnel.
