OBJECTIVES: To determine the frequencies of the transmission of HIV-1 and HIV-2 from mother to child. PATIENTS: All newborns to HIV seropositive mothers born in 62 french hospitals are included in the French Prospective Study and followed prospectively.
METHODS: 1) The positivity for HIV-1 or HIV-2 is determined at delivery by using Western-blot analysis and eventually specific peptides immunoassay (Peptilav: Diagnostics Pasteur). 2) The criterion for HIV infection is the presence of specific antibodies at 18 months of age. 3) Viral cultures were performed: HIV-1 detection was done by p24 antigen test on supernatants, HIV-2 detection by measure of reverse transcriptase activity.
RESULTS: 1) The French Pediatric Study includes now more than 1,300 children born to HIV-1 mothers and 47 born to HIV-2 mothers. 2) 528 babies born to HIV-1 mother older than 18 months of age have a known serologic status: 99 are seropositive (18.8%) and 429 have seroreverted. 3) On the other hand, 25 babies born to HIV-2 seropositive mothers, older than 18 months of age, have a known serologic status: all have seroreverted. Moreover, 67 cultures performed for 30 children born to HIV-2 mothers have been all negative (the sensitivity of HIV-1 culture in our lab is 95% for infected children after 1 month of age). So, none of the 25 children born to HIV-2 mother is infected.
CONCLUSIONS: In this study, the HIV-1 materno-foetal transmission rate is evaluated at 18.8 (+/- 3.3) %. The HIV-2 materno-foetal transmission rate is significantly lower (p less than 0.05).
