OBJECTIVE: This paper reviews recent developments in the HIV epidemic as it affects men who have sex with men, focusing on community-based efforts to reduce the impact of the epidemic and to prevent HIV transmission.

METHODS: The paper draws on the experiences from developed and developing countries, based on interviews with key informants as well as a review of available published reports.

RESULTS: There have been different responses to the HIV epidemic among men who have sex with men, depending mainly on the socio-cultural and political environment. While there are significant cross-cultural differences, there are also many common problems that need to be addressed.

DISCUSSION AND CONCLUSIONS: The HIV epidemic among men who have sex with men will require community-based efforts that consider local conditions and resources.
