OBJECTIVE: To show an unusual corneal infection in an AIDS patient that mimics keratitis sicca and its treatment.
METHODS: Retrospective chart review of a patient with corneal infection who was followed prospectively by the authors.
RESULTS: M.S. is a 37 year old AIDS patient who presented 4/95 with severe pain in both eyes. Slit lamp examination revealed severe, diffuse superficial punctate keratitis consistent with severe dry eyes (keratitis sicca). Over the next 2 weeks there was no improvement with punctual plugs increased ocular lubrication, and bandage contact lens. A diagnostic and therapeutic corneal scraping revealed microsporidiosis. Topical FUMAGILLIN therapy was indicated. The patient remained disease-free until his demise 4 months later.
CONCLUSIONS: 1) Microsporidiosis should be considered in a differential diagnosis of corneal infections in patients with full blown AIDS. 2) Treatment consists of debriding the corneal epithelium and topical FUMAGILLIN therapy.
