Astrocytoma and squamous cell lung carcinoma association in AIDS patients.

Case Report: D.P.M., 71 years old, male, white, widower, retired, bisexual, has presented AIDS for 4 years, with previous treatments for PCP, using zidovudine and TMP+SMX. One box per day smoker since adolescence, low alcohol user, no IV drug use. He presented low-differentiated squamous cell carcinoma in right lung, thus a well-succeeded radiotherapy, obeying to the following aspects was performed 5000 cGy over the tumoral area and 4500 cGy over mediastinum, supra and infra-clavicular areas, using a 4 MV linear accelerator, divided into 5 weeks. Zidovudine and clinical control were maintained. 3 months later, the patient presented recurrence, radiologic and clinical impairment, and an etoposide-carboplatin chemotherapy was indicated. Post-chemotherapy, he developed into no neurologic sign, but motor ataxia. A brain C.T. showed hypodense expansive lesion with contrast ring enforcement 38 x 30 mm, corticalsubcortical in right occipital lobe, with circumscribed edema and mass effect over the homologous occipital horn, a brain biopsy revealed level II astrocytoma, liquor presenting hypergamaglobulinemia, anti-HIV-positive and neoplastic cytology negative. Radiotherapy was initiated over occipital-parietal region with linear accelerator: 4500 cGy divided into 5 weeks and a half, as a result of a non-indicated surgery. As soon as the radiotherapy ended, he developed into a respiratory infection and death.Comments: (1) CNS primary tumors are 2 % of adulthood tumors. Relationship between glial tumors and HIV hasn't been identified yet. (2) Relationship between glial tumors and AIDS cases are rarely reported and we didn't find associated glial tumor + AIDS + second site tumor, but Kaposi's sarcoma and Lymphoma which are considered by the CDC as AIDS-Related.
