Etiology of placental membrane inflammation among HIV seropositive and seronegative women in Uganda.

OBJECTIVE: To study the factors associated with placental membrane inflammation among HIV seropositive women delivering in Mulago Hospital in Kampala, Uganda.

METHODS: A nested case control study was conducted among a cohort of 204 HIV seropositive and 181 randomly selected HIV seronegative women delivering in Mulago Hospital. Information collected at the enrollment visit at 28 weeks of gestation included socio-demographic characteristics, previous obstetric history and obstetric examination. HIV and syphilis serology were also performed and placentas examined histopathologically. Placental membrane inflammation was defined as inflammation of the membranes (chorioamnionitis) and of the umbilical cord (funisitis).

RESULTS: On bivariate analysis, factors associated with chorioamnionitis included first pregnancy (OR=0.53; 95% CI=0.31-0.89) and RPR positivity (OR=0.30; 95% CI=0.07-0.91) while factors associated with funisitis included spontaneous abortion (OR=1.90; 95% CI=1.03-3.49) and male baby sex (OR=0.45; 95% CI=0.27-0.75). On regression analysis, the following factors were independently associated with chorioamnionitis: age group 25-29 (OR=2.84; 95% CI=1.30-6.22), a history of herpes zoster (OR=5.59; 95% CI=1.05-29.89), and RPR positivity (OR=0.26; 95% CI=0.08-0.83). Factors independently associated with funisitis included: spontaneous abortion (OR=1.97; 95% CI=1.10-3.53), pregnancy related problems (OR=1.96; 95% CI=1.04-3.71) and male baby sex (OR=0.55; 95% CI=0.31-0.98).

CONCLUSION: Placental membrane inflammations are not associated with HIV infection in women in this population. Funisitis is independently associated with obstetric factors.
