INTRODUCTION: It has been reported both benefit immunology, antineuplasic effects by the application of IL2 in high doses: 5 - 22 millions of Ul SC or IV, and adverse effects: mielotoxic, respiratory, cardiovascular and renal ones.
OBJECTIVES: It is intended to evaluate if the low doses application of about 1.5 to 2.0 m of Ul of IL2 for three consecutive days every two months in adult patients VIH+, in all stages of the evolution (C.D.C. 1,2 3 and a,b,c) previously controlled with combined antivirals, it is able to raise the counts of CD4, CD8 antineoplasic activity and thus evitate the toxic effects with higher doses. METHODS: 60 VIH+ patients were studied and divided in four groups, in base to the CD4 count: GROUP 1: (3 patients), more than 500 CD4; GROUP 2 (6), de 200 a 499 CD4; GROUP 3 (32) , from 20 to 199 CD4 and GROUP 4 (19), from 1 to 19 CD4/ml subjected to three consecutive cycles in 3 days using sc from 1.5 to 2.0 M of IL2 Ul, every two months and evaluating adverse clinic effects such as cardiovascular, respiratory, hematology, renal and immune ones, determining CD4, CD8, CD4/CD8 index, total lymphocyte, clinic evolution, antineoplasic activity and viral load by means of HIV RNA and HIV DNA amplifications by chiron. Cardiovascular, respiratory and renal effects will be treated in other item. Clinical, viral load and immulogy effects are next presented. It was considered average age, adverse and beneficent effects in the clinical evolution and the indicated determinations.
RESULTS: CD4 raises in the four groups in a base of 50% within 60 days; in 7 patients from groups 2,3 and 4 CD4 remained stable or even come down 10%. A growth of +33% in all groups was reached within 60 days: no changes were registered in the CD4/CD8 ratio; total lymphocyte count raised 35% in all groups; previous viral load in 47 patients was less than 10,000 meq remaining constant in 46 and raising from 10,000 to 35,000 in just one patient, in two cases a patient come down from 45,000 to 10,000 and the other one raised to 60,000. In a patient his previous viral load was 150,000 and raised to 250,000 within 60 days and the antiviral treatment was changed. Among the beneficent effects is the raising of weight: 5 kg in 80% of all patients within the 60 days, karnofsky raised 10 points at the end of the cycle, it was found improvement as an answer to the SK treatment and the same to lynfoma in 3 cases. Adverse effects: fever in 21 cases (75 incidents), flu sky lyke syndrome in 10 patients, asthenia in 40 cases. Beneficient respiratory effects: raising in saturation of 02 and spyrometry 80% in 40 cases with damage in one case and without variation in 9 cases. Cardiac Alterations: isolated ventricular arrythmia in two patients without previous cardiopathy, controlled with xylocaine, no one patient showed renal changes.
CONCLUSION: If the antiviral treatment has previously controlled the viral load, immunology effects are very favorable, raising respiratory function and easy to control symptoms. But if the viral load is higher than 10,000 it may raise even more with the application of IL2 and deteriorate the respiratory function. Arrythmiae are caused by extension in QT segment. No serious complications were detected. So it seems to be that high doses of IL2 are not necessary, doses above adviced in this item are enough in cycles of 3-5 days to get important benefits and less adverse effects.
