Histoplasmosis in HIV patients at a non endemic zone in Brazil.

ISSUE: High prevalence of histoplasmosis in AIDS patients in the state of Sao Paulo, Brazil, "a non endemic zone".

PROJECT: Survey all histoplasmosis (HP) cases from Sao Paulo notified in the Epidemiologic Bulletin of the Ministry of Health in Brazil (EBMHB), AIDS/STD National Program that controls notification of AIDS patients in the country (Criteria OPAS -Caracas /since September 1992).

RESULTS: HP cases notified in the EBMHB: from 1980-84: among 306 AIDS cases/1 HP case (0,3%); from 1985-87: among 6.161 AIDS/34 HP (0,6%); from 1988-95: among 77.650 AIDS/840 HP (1,1%). Total: 84.117 AIDS cases and 875 HP cases (1%). From 1984 to 1995 Sao Paulo had 121 HP cases (0,14%).

LESSONS LEARNED: We were unable to verify epidemiologic risk and provenance in the Epidemiologic forms (EF) surveyed from the Ministry of Health in Brazil. These data are very important to suspect of endemic diseases like histoplasmosis, paracoccidiodomicosis and coccidiodomicosis that determine AIDS cases (CDC 1987 criteria). We emphasize the inclusion of these data in the EF because some are notified outside their place of origin (patients travel the world looking for treatment and help).
