OBJECTIVE: To determine if HIV replication increases in peripheral blood of seropositive patients after Influenza vaccination.
METHODS: Tcells and HIV RNA levels were drawn prior to Influenza vaccination in 86 HIV patients (CD4 5 - 615, mean of x 186). Patients were immunized with a one (1) time dose of .5 cc IM of the 1995-1996 formula of Purified Surface Antigen Vaccine, Trivalent, Types A and B. Tcells (CD4, CD8) and HIV RNA were again drawn at one (1) month and two (2) months following vaccination Sixty-three (63) of the patients, were on combination antiretroviral therapy.
RESULTS: (table: see text) Mean HIV RNA levels increased approximately 5 LOG one (1) month after vaccination but increases were not sustained at two (2) months. Mean CD8 count showed no significant changes at 1 and 2 month $ vaccination. Mean CD4 counts increased by 67. The highest HIV RNA increase were seen in CD4 greater than 200.
CONCLUSIONS: These results suggest that immunologic (antigenic) stimulation may result in transient increases in viral load. In considering the risk of temporary viral load increase, the risks to the non-vaccinated patient should also be considered. The potential consequences of infection may be more deleterious to the patient than a transient increase in viral load.
