BACKGROUND: Since 1998 HIV-infection widely spread in Moscow Region of Russia (MR). On 01.01.98 in MR were registered 618 HIV-infected persons, on 01.01.99 - 1215, on 01.01.2000 - more than 10000. About 90% of new cases HIV-infection is IVDU. The frequency of HCV markers in HIV-infected patients (pts.) in MR remains unclear.
OBJECTIVE: To evaluate the anti-HCV frequency, genotype, viremia, histological features in HIV-infected pts. in MR.
METHODS: During 1999 we studied anti-HCV (ELISA) in 320 HIV-infected pts., RNA HCV (qualitative PCR) in 78 pts. with anti-HCV, HCV viremia (semi-quantitative PCR) in 64 pts. with RNA HCV, HCV genotype in 57 co-infected (HIV + HCV) pts. and histological features (Knodell index and grade of fibrosis) in 16 co-infected pts.
RESULTS: Anti-HCV were obtained in 90.5% HIV-infected pts. 76 of 78 co-infected pts. (97.4%) had RNA HCV. 73 of 76 pts. with RNA HCV were IVDU. High concentration of RNA HCV (5+) was obtained in 31.3% pts., middle concentration (3-4+) - in 57.8% pts. and low concentration (1-2+) - in 10.9%. Genotype 3a was the most frequent in this cohort - 63.2%, 1b - 19.3%, 2a - 7%, 1a - 3.5%. 4 pts. (7%) had HCV mix genotype - 1b + 3a. Middle Knodell' index was 5.3 é 0.6. Minimal histological activity was obtained in 3 pts., low - 11 pts., middle activity - 2 pts. Middle fibrosis' grade was 1.0 é 0.26. 7 pts. hadn't fibrosis, 3 pts. - low grade of fibrosis, 5 - middle grade and 1 - high grade.
CONCLUSIONS: More than 90% HIV-infected IVDU in MR have HCV-infection. About 90% co-infected pts. had middle and high concentration of RNA HCV. The most frequent HCV genotype was 3a. 81.3% pts. had low and middle histological activity and 50% pts. - low and middle grade of fibrosis.
