ISSUES: The proportion of HIV/AIDS patients that become terminally ill or bedridden in Uganda is very high.If Opportunistic Infections are not adequately treated, the likelihood of being ill is high, in the long run these patients lose their jobs as constant abseteeism from duty is also high.They therfore cannot meet their Hospital bills and so gome to die!

DESCRIPTION: (a) This paper will look at ways of designing a good Homecare programme within our settings i.e rural and urban (b) Outline the qualities of a good Homecare programme Including training in Palliative care as most of these patients are either Terminal or bedridden. (c) Will design a model of the Health care Units and the persons responsible for the Homecare service. (d) Design a support supervision programme for our patients by trained community members.This will include Spiritual care, and the care for Intra venouc lines and use of IV Antibiotics. (e) Outline the source of funding and sustainability of the programme. (f) Lay out strategies for end of life issues and prepare for death. Unlike countries in the West some countries in Africa have no Medical Insurance policy for its Nationals so all HIV/AIDS patients have to meet their medical needs! This gets harder as the patients grow weaker.

CONCLUSION: The use of IV Antibiotics to terminally ill patients in a Home based care system is possible and where it has been applied patients have benefited greatly they have been able to go back to work and lead normal lives thereafter.
