Lipid profiles in untreated HIV infected asymptomatic patients.

BACKGROUND: Changes in HIV infected lipid profiles have been described in literature before the introduction of protease inhibitors. The objective of our study is to evaluate the lipid profile of asymptomatic and untreated patients in our area.

METHODS: Case-control study. Fasting serum samples of 40 asymptomatic HIV infected consecutive patients under no antiretroviral treatment, seen as outpatients in our Hospital in 1996 and 1997 and 21 seronegative control subjects were analysed for lipid and lipoproteins profile, along with immunological parameters.

RESULTS: Significant differences in means (controls vs. cases) were found in: HDL-cholesterol (52 mg/dl vs. 38 mg/dl, p>0,0002); LDL-triglycerides (41 mg/dl vs. 63 mg/dl, p>0,012); total cholesterol / HDL-cholesterol (3,4 vs. 4,5 p>0,006); apoA (158 mg/dl vs. 127 mg/dl, p>0,0001); apoB100 (96 mg/dl vs. 86 mg/dl, p>0,05), haematocrit (45% vs. 42%, p>0,05) and total lymphocytes (2450/ml vs. 1668/ml, p>0,004). In the case group LDL-cholesterol correlated to immune activation markers (beta-2-microglobulin, p>0,05 and viral load, p>0,01).

CONCLUSIONS: In HIV-positive we observed, compared to controls, a significant lower serum HDLndash;cholesterol and a change in LDL composition pattern, with lower cholesterol and apoB100 content and higher triglyceride content, suggestive of altered synthesis of LDL in the liver.
