BACKGROUND: The optimal approach for diagnosis Primary HIV-1 Infection has not been defined. We intent to describe one case of Primary (HIV) Infection in a patients who had studies and symptoms consistent with Syphilis and Infectious Mononucleosis.
OBJECTIVE: To determine the usefulness of symptoms and virologic test for diagnosing Primary HIV-I Infection.
METHODS: A woman (17 years old) was raped 60 days before hospital examination. She presented an infectious and viral syndrome: Fever, Myalgia, Rash, and arthralgia . It was tested Antibodies(Abs.) (Abbott,MEIA 3.0) Confirmed by Western Blot ( BIOBLOT HIV-1 PLUS), P24 Antigen(Ag.) (Org. Tek., Co. Biomerieux Group) and Viral RNA(PCR) for HIV.
RESULTS: 1* Sample Abs. Borderline (S/Co= 1,31) and Ag. (5600 Pg./ml). 2nd. Sample (3 days later): Abs. Reactive (S/Co= 9,43) and Ag.( 1920 Pg./ ml). 3rd. Sample (4 days later): Abs. Reactive (S/Co= 12,42) and Ag.(1120 Pg/ml).4th. Sample (5 days later): Abs. Reactive (S /Co. 11,74) and Ag.(160 Pg/ml). Sensitivity and Specificity of the P24 Antigen were 88,7% (95%CI, 77,0 to 95,7) and 100 (CI, 99,3 to 100) respectively, for MEIA Sensitivity 100% and Specificity 99,94%. The Viral Load (Amplicor HIV- MONITOR 1,5 ROCHE) detected 4.967.968 Copies /ml. -6,7 log10 at 3rd. Sample. Sensitivity 100% and Specificity 97,4% (CI, 94,9 to 98,9).
CONCLUSIONS: Clinicians should recognise that a negative result of antibody test doesn't exclude the presence of Primary HIV Infection. In addition to this, is important to perform an assay which can detect the P24 Antigen and the viral RNA (PCR) in those patients who have an active sexual life and present characteristic symptoms of early infection.
