BACKGROUND: Studies carried out in different populations reported association between some HLA specificities and susceptibility to tuberculosis. The aim of this study was to investigate the class I and class II HLA profile in Brazilian patients with AIDS and tuberculosis.
METHODS: Twenty-two adult patients with AIDS and tuberculosis (Tb); 103 patients with AIDS without tuberculosis (NTb) and 423 healthy individuals not infected with HIV (C), from the same geographic region and of similar ethnic background were evaluated. The diagnosis of tuberculosis was performed taking in account the clinical/radiological presentation and the results of direct baciloscopy or culture of appropriate clinical specimens. Class I antigens were typed by microlymphotoxicity. Class II alleles were characterized by the polymerase chain reaction (PCR). The frequencies of class I and II HLA specificities were compared by chi-square or two-tailed Fisher exact test (significance level: 0.05). The odds ratio (OR) and the Cornfield confidence limits (CI) for OR were also calculated.
RESULTS: Differences in frequency of HLA specificities between groups were found in the following antigens/alleles: 1) Tb X NTb - HLA-A31: p=0.025; OR=3.92; CI=1.06<OR<14.37; HLA-B41: p=0.038; OR=7.97; CI=0.99<OR<74.31; HLA-DRB1*10: p=0.038; OR=7.89; CI=0.98<OR<73.53; HLA-DQB1*05: p=0.0093; OR=3.83; CI=1.33<OR<11.23; 2) Tb X C - HLA-A31: p=0.0000024; OR=42.33; CI=7.99<OR<250.01; HLA-B41: p=0.0038; OR=20.13; CI=2.59<OR<186.07; HLA-DQB1*05: p=0.021; OR=3.17; CI=1.16<OR<8.85.
CONCLUSION: The HLA-A31 and HLA-B41 antigens; and the HLA-DRB1*10 and HLA-DQB1*05 alleles were over-represented in the Tb group, suggesting that these HLA molecules might be associated with susceptibility to tuberculosis in Brazilian patients with AIDS.
