Putting in place blood algorithm based on a speedy tests for aids screening.

ISSUE: in 1998, a report from West Cameroon health centres said that a dozen of speedy tests have been performed. Prices of those tests varied from one centres to other. No guarantee for wholesome blood transfusion. This conduct us to carry a study aiming at identifying 2 speedy tests ( sensitive, trustworthy, easy to handle and keep and much cheaper) for HIV screening.

DESCRIPTION: - sensitisation of health centres bosses for the above mentioned reasons; - taking stock of tests circulating within the West province - getting in touch with Centre Pasteur in view of determining tests to confirm; - submission of reactives proved effective for evaluation; - choice of tests for screening, first and second intention; - meeting with laboratory representatives so as to come to a compromise on how to harmonise reactive prices for a touchable level; - training of laboratory specialists; - application of the algorithm; - follow up and evaluation.

ISSUES: -12 speedy tests have been numbered in health centres and handed over to Centre Pasteur in Yaounde; - 4 most used tests are proven to be qualified by the majority; - CAMTIX and IMMUNO COMB II have been retained for first intention screening, and HIV SPOT and ABBOT DETERMINE for second intention screening; - we had availed ourselves of the chance to reduce the cost of screening from 11 to 5,5 US dollars through negotiations; - a document from health delegate urging health centres personnel to apply the algorithm has been conveyed to district centres bosses; - 22 out of 25 centres are applying already the algorithm and the number of tests circulating has gone down from 12 to 5.

RECOMMENDATIONS: the first algorithm yielded good results on individual screening. The second algorithm to be evaluated; based on wholesome blood transfusion, for a complete study.
