Study of the gastrointestinal tract diseases in hiv-patients in Lima-Peru.

BACKGROUND: Determine the ethiology of the syntomatic disease of the upper gastrointestinal tract in HIV-infected patients to guide the selection of a most appropriate empiric therapy.

METHODS: The prospective and analytical study includes HIV infected-patients between 18 and 70 years old (100) that were sent to upper endoscopy. Patients with dysphagia or odynophagia get a test with Ketoconazole for 3 days. These samples will be obtained: esophageal biopsies and cultures in focal diseases, aspiration of the gastric content to pH and Mycobacterium and H.Pylori, antral biopsies for histopatology, Mycobacterium and H.Pylori., duodenal aspiration: parasites, BK, micosporidium, Mycobacterium. Duodenal biopsies:histopathology,Mycobacterium. Dosage for CD4.

RESULTS: This study was done between January 1998 and February 1999. 25 of the cases with the following endoscopic

RESULTS: esophagitis (17), gastritis (21),duodenitis(16),duodenogastric reflex (9), erosive antritis(2),active antral ulcer(1), Yamada III high lesion (1). Histopathology: In the esophagus 1 case of virus Herpes and of Anbicans Candida; in the stomach 6 cases od H.Pylori,, 1 of Cryptosporidium and 4 of gastric atrophy. Culture of Biopsy: 2 cases of Mycobacterium and H.Pylori. Cultive of Gastric Juice: 1 case of Tuberculosis M.

CONCLUSIONS: It was observed cases of Herpes virus, cytomegalovirus, candidiasis, through biopsies just with inflammatory changes in the endoscopy. There were found cultures to positive M. Tuberculosis, gastric and duodenal juice not reported in other series. No discovery of common parasites would be explained through various treatments that the patients received.
