BACKGROUND: Tuberculosis (TB) and AIDS are great problems of public health in Brazil. The state of Rio de Janeiro has high prevalence of both disease. Here we describe the patterns of co-infection in a reference center for HIV treatment (Centro Previdenciario de Niteroi) wich is located in metropolitan region of State of Rio de Janeiro, Brazil.

METHODS: Database of TB Control Program and medical records.

RESULTS: The reference center takes care of about 500 patients per year. 96 cases of TB were notified from HIV patients from 1995 to 2000. The incidence and distribuition of the disease per year doesn't change even after HAART. Patients had from 6 to 63 years old and 71,9% were male. Diagnosis: 15,5% with positive acid fast bacilii; 12,9% with histopatological finds and 6,9% with positive culture.70% had abnormal chest XRay. 53 patients had extrapulmonary disease (45% in linfonodes; 9,4% pleural ; 7,5% miliary and 7,5% with meningitis). 23,5% were hospitalized and the main indication for hospitalization was caquexia. 15% were TB contacts. 93% of the patients begins treatment with drugs of first line in Brazil (RHZ).

CONCLUSIONS: TB is still an important oportunistic disease associated with HIV even after HAART speccialy in regions of high prevalence of TB. Bacteriological diagnosis is poor in this sample. 15% of patients were TB contacts wich can means fail in TB prophilaxis. An important amount of these patients needs hospitalization wich reminds the special attention with precautions with airbone transmission in this kind of reference center.
