Pneumocystes carini pneumonia in aids cases in Mumbai, India.

BACKGROUND:- Number. of HIV infected individuals progressing to AIDS with apportunistic infections is incrising in Mumbai, India. This report discribs uncommonly reported Pneumocystis carini, pneumonia, in AIDS cases in Mumbai.

METHODS:- induced sputum samples and braonchoalveolar lavage (BAL) samples from 214 clinically suspected Pneumocystis carini pneumonia in AIDS cases and other immuno commpromised condition were studied. 170 patients were HIV positive and 44 were HIV negetive. Smears from sputum and BAL samples were stained with TBO, GMS & Giemsa. In 30 positive smears immunofluorence for P carini was carried out using commercial kit for both trophozites and cysts

RESULTS:- Mejority of patients 158/214 were between the age group of 21 to 40 years. 77% were males and 23% were females out of 214, 54 (25%) smers were positive for P carini. 40/170 (26%) HIV positive patients and 9/44 (20%) Immuno commpromised patients were positive for P carini. The predominent morphological form observed was trophozoite in 76% of the cases. The smear positivity by indivisual stains was around 15 to 17% but if all three stains are used the positivity can be improved to 25% All 30 smears were confirmed prescence of trophpzites and cysts by immunofluorscence

CONCLUSION: - Pneumocystis carini, pneumonia is a common respiratary tract aportunistic infection. Combination of three stains TBO, Giemsa & GMS gave best results. These simple stains are of great use in the diagnosis of Pnemocystes carini pneumonia in the rsource limited countries
