INTRODUCTION: In the pre-HAART era, it no was infrequent that patients with a new diagnosis of HIV infection had a bad immunological or clinical situation at the presentation. It could be possible that the news about the efficaccy of HAART favours to be in a better situation at the presentation.OBJECTIVES: To describe and compare the clinical and immunological situation of the newly diagnosed patients of HIV infection (NDP-HIV) in the last six years.
METHODS: Prospective study of all NDP-HIV between 01/01/1995 and 31/12/2000 in the area served by our Unit (near 700.000 people).
RESULTS: We observed 335 cases of NDP-HIV in the 6-years period. The risk practice was: IVDU (40,6%), heterosexual (31,6%), MSM (12,8%), unknown (13,7%) and bood transfusion (1,2%). 228 (68%) were male and 107 female (32%). The mean of CD4 count at the presentation was 360,53 (IC 95%: 328-393). In 110 cases (32,8%) the patients had a opportunistic condition of C category at the presentation. Female presented in better inmunological condition ( "earlier") than male (CD4: 404 vs 339; p=0,068). MSM patients presented also earlier than patients with other risk practices (CD4: 474 vs 341 in heterosexual -p<0,05- and 347 in IVDU -p<0,05). There was no differences in the inmunological and clinical situation between NDP-HIV in the preHAART (1995-6) and the postHAART (1998-2000) era: CD4 375 vs 333; concomitant diagnosis of AIDS: 33 vs 27%.
CONCLUSION: The information in the media about the efficacy of HAART therapy, have not influenced in an earlier presentation of the patients. The presentation is later in patients IVDU or heterosexual than MSM patients. Female seems come up earlier. This results suggests that it is neccesary to persevere in the information about the usefulness of making HIV test in patients with risk practices, (specially IVDU or former-IVDU and people with history of unprotected sexual contacts), in order to make a early diagnosis..
