BACKGROUND: The most common ocular opportunistic infections in patients with AIDS are due to exacerbation of latent infectious processes acquired before the immunosupression. Study carried out in our Institution before the free availability of HAART in Brazil showed that the most prevalent ocular complication in AIDS patients in the southeast region of this country was CMV corioretinitis (40%) and 81% of the patients presents ocular alteration. The objective of the present study was to compare the profile of ocular disease after the advent of HAART in patients with AIDS with free access of this medicines, provided by the Ministry of Health for all Brazilian patients with AIDS indicated to receive this treatment.

METHOD: The study population was composed by 200 patients with AIDS followed-up at the University Hospital of the Faculty of Medicine of Ribeirao Preto, SP, Brazil, and the ophthalmic evaluation was performed by the measurement of visual acuity (Snellen table) and indirect binocular ophthalmoscopy.

RESULTS: ocular problems were present in 18% of the patients and the most prevalent infectious ocular complication was toxoplasmosis chorioretinitis (6,5%), followed by cytomegalovirus retinitis (4%) and other infectious processes (2%).

CONCLUSION: These data show a decrease in ocular involvement and a change in the profile of associated ocular diseases.
