Survival analysis of pediatric AIDS in Brazil.

BACKGROUND: In Brazil, the survival time of AIDS cases in the period 1982 to 1989 was 5.1 months(mo) and an another study for the period 1985 to 1991 showed a time of 201 days(6,7 mo)for mother-to-child transmission(MTCT). Knowledge of survival time subside indicators to evaluate strategies to prolong life expectancy of AIDS patients.

METHOD: Cases: AIDS in children (0-12 years), reported to the surveillance system from 1983 to 1998. Design:Cohort analytical study with secondary data. Entered the study: 1048 children diagnosed up to December-31-1998 and whose reporting forms reached the system by December-31-1999. They were followed up to June-30-2000. Statistical

METHODS:Kaplan-Meier and Cox proportional hazard analysis. Main outcome measures: Time in mo elapsed from birth(DOB) to diagnosis(DDX) according to year of birth; eighteen-month survival time after DDX, according to gender, year of DDX, and age at DDX. Result(primary findings):In 1048 cases, median time of survival from DDX was 43.8 mo for boys(n=588) and 56.3(n=460) for girls. Of 824 cases of MTCT, median time of survival from DDX was 60.8 mo for boys(n=418) and 64.8 for girls(n=406). In this group, survival time from DOB was 102.1 mo for boys and 112.5 mo for girls; the median time from DOB to DDX decreased from 30.8 mo for those born in 1983 and earlier, to 2.2 mo for those born in 1998(p=0.0000); for death in 18-months, the HRs(95% CI) considering year of DDX were: 1983=1.00; 1983-91=0.58(0.36-0.94); 1992-94=0.43(0.23-0.68) and 1995-98=0.32(0.27-0.69).

CONCLUSION: In Brazil the policy to assist HIV infected people includes: free access to antiretroviral therapy; prophylaxis for opportunistic infections and access to day care and home care assistance. This policy has provided a longer survival and better quality of life which is related to better physical and emotional conditions. Then, the children infected with HIV are entering adolescence at increasing number.
