The epidemiological status of IDU and HIV infection in addict cases submitted to HIV/STI/IDU Counseling and Care Center in Kermanshah province.

INTRODUCTION: The assessment of The Kind of drug abuse and the pattern of use and history of prison is very important for evaluation of trend of HIV in the future.

METHODS: In this study volunteer addicts were evaluated and IDUs were tested for HIV.

RESULTS: Of 731 volunteer addicts, 731 were male (97.5%) & 18 female. 2 cases <15y(years), 18 cases between 15-25y,423 cases (57.9%) between 25-40 y and 187 cases(25.5%) >40 y with mean 34.6y.(14-72 y).335 cases (45.9%) were single, 286 (39.1%) married, 87(11.9%) divorced 19(2.6%) widow and 4 unknown. 506 cases (69.2%) had history of prison, with mean 29.7 months (1-360months). The recent method of abuse in 222 cases (30.4%) was smoking, 90 cases (12.3%) snuff, 314 cases (43%) injection, 45 cases (6.1%)oral, 39 cases (5.3%) not use and 21 cases (2.9%) was unknown. 105 cases (14.4%) used morphine, 599 cases (76.4%) Heroin and 27 cases (9.2%) other substances. 429 cases (58.6%) had history of IDU and 302 individuals (41.3%) had needle sharing. The mean of injection during was 12.8 y.(1-52 y). 19.2% of tested cases had HIV infection. The most common infected cases age group was 15-25 y.25% were divorced & 16.9% single. The chance of HIV infection in previous prisoners was 2 folds specially in 5-10 years (34%) residency. 30% of oral addicts with previous injection history had HIV infection. The primary (31.2%) and illiterate (28%) cases had the most exposure of HIV infection. 55% of addicts with age beginning <15y were injecting and 20% had HIV infection, compare to 12.5% >40 y were injecting and nonHIV infection.

DISCUSSION: Injection form(56.8%) & Needle sharing (41.3%) in addicts of our province is very high. The heroin using is very common (76.4%) which can be abuse IV. Most of the addicts have low level of knowledge and more than half of them started addiction before 20 y, so education of young people with low level of knowledge is very important for prevention of HIV infection.
