Successful nevirapine-based prevention of mother-to-child transmission in Southern Ukraine.

BACKGROUND: There is a paucity of data about the effectiveness of nevirapine-based prevention of mother-to-child transmission (PMTCT) programmes in field situations in resource-poor settings. Medecins sans Frontieres, in co-operation with the Ministry of Health of Ukraine, runs a PMTCT programme in Southern Ukraine, in a population where 30% women have a history of injecting drug use and where prevalence may be as high as 3.7%.

METHODS: All HIV-positive mothers in labour in the maternity clinics of 3 Southern Ukrainian cities were offered nevirapine. Women received a tablet of 200 mg of nevirapine during labour and newborns received 2mg/kg syrup of nevirapine suspension within 72 hours after birth. Mothers were supplied with infant formula. After an average period of 3 months children were tested for HIV by RNA polymerase chain reaction (PCR).

RESULTS: From July 2000 untill December 2001, with 193 mother-child pairs in the progrramme, PCR data are available for 128 mother-child pairs. General transmission rate is 11.7%; this includes partially treated mother-child pairs and also mother-child pairs of whom the mother was given a partial or full course of AZT from another programme. The transmission rate among mother-child pairs receiving the full nevirapine protocol is 7.4% (n=68). In those receiving nevirapine and AZT the transmission rate is 11.1% (n=36). Among those who also had and elective caesarean section the transmission rate is 3.3% (n=60).

CONCLUSIONS: It is effective to prevent mother-to-child transmission of HIV using nevirapine-based regimens in Southern Ukraine in a setting with limited resources and in a population with a significant proportion of injecting drug users. Updated figures will be presented.
