BACKGROUND: Parlem Clar is an HIV & pregnancy prevention program for high school youth (17 y). It promotes condom use among students who have sex and access to condoms. It is peer delivered. It was developed in 2000, based on Intervention Mapping methods, identifiing personal and environment determinants and exclusively promoting their change.
METHODS: Community intervention trial. Participation was offered to all Barcelona high school centers. Those accepting to participate were randomly assigned to intervention and control conditions. 10 centres reached inclusion criteria (601 students). Baseline data were collected in December 2000 and follow-up data 4 months later. 442 questionnaires were linked (74%). Knowledge on sexual intercourse, condoms and comunity ressources, attitudes, social norm, self-efficacy and some relevant behaviors were analysed through multivariate analysis adjusting for baseline data, sex or other relevant demographics.
RESULTS: At follow-up, students in intervention schools increased knowledge on transmission (saliva (p<0,001), glasses (p<0,001), bloody tools (p<0,05)), treatment (p<0,001) and community ressources. It increases susceptibility (p<0,05), severity (p>0,05), self-efficacy on buying condoms (p<0,05), negotiating its use with usual partner and others (p>0,05). Self-efficacy and intention to refuse nonprotected sex among boys and benefits of condom use increase. Preventive subjective norm increase among girls. 28% of the students have ever had an intercourse at follow-up. Parlem Clar doesn't promote earlier sexual intercourse. Condom use at last intercourse increases (p<0,05).
CONCLUSIONS: Parlem Clar changed psychosocial determinants and behaviors. A short intervention, based on peer education and promoting access to condoms is feasible and effective in a southern european city.
