Linked networks of service: step-by-step development of a technology based adherence program.

ISSUES: Non-adherence to referrals and treatment plans for clients at high-risk for HIV, sexually transmitted diseases and TB is a problem for human service agencies working in medically underserved, low-income Chicago communities. An electronic, linked network of services (LNS) is needed to address this issue.

DESCRIPTION: By integrating a computerized case management software system, a two-way message routing software, and wireless technologies (i.e. wireless notebook PC's, cell phones, and pagers), our Technology Based Adherence Project (TBAP) creates a wide area network focused on improving patient adherence. Providers electronically share key patient information to facilitate case coordination, patient access to services and continuity of care. Patients receive electronic, individualized, interactive reminder messages via a preferred method of communication to increase adherence to established treatment plans. This inlcudes medication and appointment reminders for example.

LESSONS LEARNED: In this session we will present initial outcomes from TBAP, paying particular attention to the process knowledge gained with respect to: Increasing patient access to care, Selecting an appropriate computerized LNS, Budgeting considerations, and Implementing a comprehensive plan.

RECOMMENDATIONS: Participants will: 1) Have a clear definition, and overview of a strong linked network of service (LNS) and a technology based adherence program. 2) Have heard "lessons learned" on how to plan for service implementation and leveraging additional resources. 3) Have completed an "Integration Matrix" exercise that will identify their level of collaboration with established or future community partners and gauge their LNS development readiness. 4) Be able to identify the type of linked network of service system most appropriate for their needs-"high tech" versus "low tech" technology based adherence program.
