Evaluation of therapeutic adhesion in patients with HIV positive.

BACKGROUND: The cause of ARV treatment failure in HIV infection is multifactorial and may involve poor adhesion, low drug levels,resistant HIV strainsand others demographic or clinical factors. This study is to determine levels of adherence and variables affecting adherence among patients enrolled in our cohort.

METHODS: Cohort were followed for 12 months to determine the rate of non adherence. Qantitative, crosssectonal study. Patients under antiretroviral therapy during at least 12 months with an inhibitor of protease and two nucleosides analogues.126 patients had been recruited. Compliance measures(%compliance,attitutes to medecine scores)drug levels, demographic factors and baseline clinical/laboratory measure were recorded at entry.

RESULTS: Patients were a median of 38 years ages and were mainly female(58%). Patientshad advanced diseasewith 63% being in CDC stage and median viralloads and CDCcount less than 350/mm3. 44 patients were recruited as failures with viral load more than 1000 copies/ml. Percentage adherence to ARV medication over the previous 7 days was good with 80%.

CONCLUSIONS: The treatment adherence is one of the most comprehensive efforts to date to explore the host of factors influencing adherence to HAART among persons with HIV/AIDS.
