BACKGROUND: The Ministry of Public Health supports antiretroviral therapy (ART) for symptomatic HIV/AIDS patients and asymptomatic HIV with CD4 counts<200 cells/muL with the regimen of GPO vir (3TC+d4T+Nevirapine). The national guidelines recommend monthly follow-up for clinical assessment and six month follow-up for CD4 count.
METHODS: Data on demographics, clinical status and response to GPO vir were extracted from clinical records of patients from six northern provinces who enrolled for HAART between September 2002 to December 2003.
RESULTS: Of 1671 cases, 58.3% were female. The mean age was 34.7 yr. and mean weight was 48.8 kg. Initial clinical assessment demonstrated 50.6% of AIDS, 36.5% of symptomatic HIV and 12.8% of asymptomatic HIV. The median baseline CD4 cell count was 42 cells/muL (0-404). Patients with visits at 6 and 12 months were evaluated and showed significant increasing of mean weight (52.7, 54.2 kg. respectively, p<0.001), and median CD4 cell count (168.5, 197 cells/muL respectively, p<0.001). One hundred ninety four patients (12%) developed adverse events while 32 cases (1.9%) had severe rash. Up to date the patient statuses are 88.9% continue treatment, 1.4% refer out, 1.5% loss to follow-up, 4.7% death related HIV and 0.1% death not related to HIV.
CONCLUSIONS: This experience is an illustration of the efficiency and acceptability of HAART with GPO vir regimen whereas the activities for the improvement of ARV drug adherence have been initiated in this area.
