INCIDENCE OF HIV INFECTION OF SEXUAL ASSAULT IN BANGKOK, THAILAND

OBJECTIVES: The Police General Hospital verified half of sexual assault cases in Bangkok, Thailand. These women and children tend to be infected with HIV. The policy to decrease half of new HIV infected cases in Thailand by the year 2010 has been set up. This study is aimed to explore the incidence of HIV infection in sexual assaults, and PEP in rape victims, as well as following up.

METHODS: From January to December 2006, the Police General Hospital Anti-AIDS program offered counseling, blood testing for HIV, HBV, HCV and VDRL in sexual assaults. PEP was prescribed if sexual contact occurred within 72 hours whereas the perpetrator was HIV infected or with high risk profile. HIVAb would also be tested again in 3-month follow-up.

RESULTS: Out of 1314 (100%) sexual assaults, 530 (40.3%) victims were children under 15 year of age. 1174 (83.5%) victims were interviewed and 1134 (86.3%) of them gave consent for HIV blood testing. 5 (0.38%) victims were HIV infected when tested. 385 (29.3%) fulfilled criteria of receiving ARV but only 141 (10.7%) were able to complete the 28-day course. Only 374 (28.5%) returned for 3-month follow-up without evidence of HIV seroconversion. This program costs USD 33,242.5 or USD 25.3 per person on top of the regular verification while average annual cost of second line regimen of NAPHA program is USD 6737.4 per person.

CONCLUSIONS: It was found that the incidence of HIV infection of sexual assault was 0.38%. Although the cost effectiveness of the program could not be evaluated, the program has lead to the deduction of HIV infection along with the rights to be protected according to the Constitution of Kingdom of Thailand.
