Program Eligibility
The patient must meet the following criteria to qualify:
  • Has no health insurance or no prescription coverage.
  • Income information must be provided.
Enrollment Process
To receive an application the doctor or patient must call. An application form will be faxed. Application forms can also be obtained from their website at www.doxiline.com
Re-Application
Application must be renewed every 6-months.
Comments
The approval process takes 24-48-hours after the receipt of the enrollee's information.
