Eligibility Criteria
- Documented HIV infection
- Documented Louisiana residence
- Documented household income at or below 200% of the Federal Poverty Level
- No coverage for protease inhibitors or antiretroviral outpatient medications. See below for Medicare Part D
- Current prescription for medication(s) covered by the ADAP formulary, written by
a licensed physician
- No financial assets exceeding $4,000 (excluding a house and a car)
Process for Certification
How to Enroll Individuals can enroll in Louisiana ADAP through one of two processes:
- Individuals who receive medical care from an LSU Medical Center (charity hospital) will be
screened for eligibility and referred by the Hospital Admission Technicians (HATs) to the
Social Services Department for enrollment in Louisiana ADAP. Eligible individuals can
often be screened and enrolled in the same day. Prescriptions will be filled by the pharmacy onsite
at the Medical Center, and Louisiana ADAP will be billed on a monthly basis for
medications listed on the current ADAP formulary that were dispensed to eligible individuals.
- Individuals who receive medical care through Ryan White Part A, Part C or from a private
sector clinician must submit an application for enrollment into Louisiana ADAP. The individual,
case manager or medical provider may submit this application. Completed applications and
required documentation should be mailed or faxed to Heather Weaver, G.S.W.,

ADAP and Medicare Part D -
Effective January 1, 2007, ADAP has coordinated with Ryan White Part A and Part B programs to provide assistance with premiums, deductibles and co-payments related to the Medicare Part D Prescription Drug Program. Eligible individuals are required to apply for the Low Income Subsidy (LIS) and sign up for a Medicare Part D plan that has been
selected by the HIV/AIDS Program in order to access medication assistance paid for by any of the Ryan White Part B programs.
Getting Meds and Services- Due to federal regulations, individuals cannot be charged a fee by a clinician's office or social
service agency for completing the ADAP application, or for submitting any other required documentation.

Completed applications will be screened for medical and financial eligibility. This process takes
approximately seven to ten business days. If determined to be eligible for Louisiana ADAP,
individuals will receive an ADAP certification card verifying their enrollment status. With this
certification card ADAP participants can utilize the services of the pharmacy at the LSU Medical
Center nearest to them. Prescriptions for
medication on the ADAP formulary will be filled at no cost for these individuals. However it is
important that medication be provided from only one Medical Center.

Individuals who receive medical care through Ryan White Part A, Part C or from a private sector
clinician are required to complete a full ADAP Recertification, including medical, residential,
financial and third party provider screening, on an annual basis. The annual medical screening
process requires that the applicant's clinician complete a medical form provided by the
HIV/AIDS Program. Eligibly recertification requires that the individual provide current
residential, financial and third party provider documentation during full ADAP Recertification
and every six months or as eligibility status changes.

Louisiana ADAP Financial Documentation Requirements
- A copy of the most recent Tax Return
- A copy of the most recent pay stub indicating the time period covered by the check (i.e. weekly,
bi-weekly, monthly)
- A statement from the unemployment office stating no record of employment
- A copy of current award letter for SSI, SSDI, Food Stamps, VA, Workers Comp, pension or
other benefit
- A copy of SSI, SSDI, Food Stamps, VA, Workers Comp, pension or other benefit, monthly
check
- A bank statement showing a monthly direct deposit of SSI, SSDI, Food Stamps, VA, Workers
Comp, pension or other benefit
- A legal document showing the amount of child support or alimony
- A legal affidavit declaring the amount of monthly cash income or a client's lack of income,
(available at any Part B Service Provider agency through a contract with AIDSLaw or in

Please note: if the applicant lives with an adult family member or a legal spouse, financial
documentation is required from all of these individuals.
Example: an adult son living with his parents counts as a three-person household,
and financial documentation is required from all three individuals.
Example: an adult female lives with a two friends/room mates, but since none
of them are family and she is not married to either one, financial
documentation is only required from the applicant
Patient Assistance Programs : Sometimes a state has a waiting list until certain programs become available. You may need to try a Patient Assistance Program, or try to get Medicaid or Medicare or private insurance first. Do not ever drop your private insurance without first seeing if your state will cover part or the whole premium, especially if you have children. The pharmaceutical industry has set up programs that offer medications for free, or sometimes at a very low price. You may have to apply directly to a specific company, or use a national service set up by a group of companies like pparx.org. Your doctor or another member of your health care team like a case manager will usually help you out to apply, and you may need to apply to more than one company for the treatments you need.
