Dr. Akhteruzzaman Molla and colleagues working at Abbott Laboratories' Global Pharmaceutical Research and Development organization in Abbott Park, Illinois evaluated the safety and efficacy of lopinavir in combination with a number of other protease inhibitors.
Lopinavir combines well with similar agents, enhancing viral suppression with limited toxicity, Molla and coauthors found.
Most of the protease inhibitors examined demonstrated additive effects when combined with lopinavir in vitro, they said. These agents included commonly used drugs such as indinavir and nelfinavir, as well as novel protease inhibitors like tipranavir and BMS-232632.
Lopinavir was particularly effective in combination with saquinavir, study data showed. The two agents displayed a synergistic relationship at all dose levels and ratios.
No lopinavir-induced cytotoxicity was seen at any of the concentrations tested (In vitro antiviral interaction of lopinavir with other protease inhibitors, Antimicrob Agents Chemother 2002 Jul;46(7):2249-53.
"These results suggest that administration of combinations of the appropriate dose of lopinavir with other protease inhibitors in vivo may result in enhanced antiviral activity with no associated increase in cellular cytotoxicity," Molla and colleagues concluded. "More importantly, the observed in vitro synergy between lopinavir and saquinavir provides a theoretical basis for the clinical exploration of a novel regimen of lopinavir-ritonavir and saquinavir."
